Isaac Jacobs, LMSW
Isaac has been working with a diverse range of children and families across the Rochester region for the past 6 years. His work has included individuals and families affected by mental health, addiction, developmental/intellectual challenges, divorce, trauma, poverty, and other challenges. Isaac approaches his work in a collaborative mindset; recognizing the importance of aligning with people to determine the best path through the challenges they bring. Isaac is also an open-minded therapist, who seeks to understand others in order to provide support and guidance in the best way possible.
Isaac enjoys working with transitional age youth, such as tweens, adolescents, and young adults. There are many challenges that can occur during these phases in addition to the typical developmental changes and they can have an impact on getting through daily life successfully. Of course, this also includes parents who struggle with how to best support their kids through these challenging times in addition to juggling the rest of life. Isaac recognizes the reality of these challenges for all involved and how important it is to have a place where parents and kids can share what’s going on without fear of judgment and where an outside perspective can make a difference.
Isaac has also worked with many youth and adults who have been through traumatic experiences, particularly various types of abuse and other experiences which have the potential to negatively alter the course of one’s life. In particular, Isaac has worked with adolescent and adult men who have been impacted by these experiences and are at risk of repeating these same patterns with those they care about most. Isaac believes that offering a safe space to begin the recovery and healing process can lead to lasting, positive change and realizing one’s full potential for their life.

KrisKristin A. Rapp, LMSW offers creative solutions to:
Create a New Life Story
Sometimes you feel stuck in a continual loop telling the same old story -- trapped by circumstances or a challenge that is getting in the way of productivity in life, relationships, or work. Through a process of self reflection and honest insight, you will align with your highest good, identify patterns that no longer serve you, affirm your worth, and build strength & self love. From this stance, you can zero in on what feels good and makes you happy. By determining your heart's deepest desires, you will discover how to fulfill these through your own thoughts and inspired action and stop getting in your own way. Wipe the slate clean and start fresh -- write a new story where you "do what you love and love what you do".
Kristin guides this process using a balanced, caring, and gentle approach, including straightforward confrontation and humor, as needed.
